When an Athlete Evaluation Request is submitted, we may collect parent or guardian name, email address, telephone number, and preferred contact method. We may also collect the athlete’s name, age, date of birth, sport, position, school or team, competition level, goals, development priorities, current training, training interests, availability, important dates, and injuries, physical limitations, or training considerations voluntarily provided.

We also collect the contents of form submissions and communications sent to FMA.

2. How We Use Information

We use submitted information to review evaluation requests, communicate with parents or guardians, understand athlete goals and needs, recommend appropriate program placement, support athlete development, schedule next steps, respond to questions, maintain records, protect the website, and meet applicable legal obligations.

Information about injuries or limitations is used only to understand disclosed training considerations. The website and evaluation form do not provide medical advice.

3. Website Operations and Analytics
